Send the packet to the nurse

The admission sheets (assessment, care plan, medication schedule…) are filled by the nurse in the patient's home. You send them all at once as a secure link to her portal; she fills, signs, and each sheet comes back to the file.

Where to find it

Open the PATIENT file → "Packet to the nurse" (top right); or Dashboard → "Packets to send to the nurse"

  1. The 60-Day RN Follow-Up Note uses one page: patient and visit details, clinical assessment, follow-up review, two notes lines, then nurse name, license, signature and date. Complete it in the portal; the signature and signing date are added when signing. Earlier requests retain their original version.
  2. Close dismisses the form immediately while pending changes finish saving. If saving fails, an alert tells you; reopen the form in the same tab to retry. Signing dates are filled on signing, not typed.
  3. On desktop and mobile, click an editable date to open the calendar beside that field. Choose a day or type MM/DD/YYYY; use the month and year selectors for older dates. Clear removes the date, and Escape closes the calendar.
  4. Open the patient's file and press "Packet to the nurse" (the number on the button is how many sheets are waiting). From the Dashboard, "Packets to send to the nurse" opens the same window for every patient at once.
  5. Check the list: each sheet shows "Admission" or "60-day renewal", its due date and its state.
  6. Choose the nurse. The assigned one appears preselected; if the patient has none, "Choose nurse…" or type her email.
  7. Optional: "SMS reminder if she does not open it" sends one text if the email is not opened in about two days.
  8. Press "Send packet". You will see "Packet sent". Each sheet turns to "Sent to …" and, when she signs it, it comes back to its row (some need the office countersignature).
Tip

"Resend email" repeats the link if she did not receive it. "Change nurse" hands the packet to another one: what is pending is withdrawn from the first.

Please note

A sheet already in her hands does not appear in the packet again, and "Renew" on that row is hidden until she finishes. The system never sends the same sheet twice.
