Using your own form

Upload your blank PDF and it becomes the paper. The system finds the boxes on it; you check them and say who signs where.

Where to find it

Open a sheet → ⋯ → "Use my form"; then Top menu → "My forms"

When your agency already works with a paper of its own — because a payer demands it, or because it has been that way for years — you do not have to rebuild it. Upload it and it becomes the sheet: what you see in the editor is exactly what will be printed, your paper, at full quality.

What to upload

  1. A form that is NOT ours — yours, or one a payer hands you. Official forms (the IRS W-9, the AHCA sheets) belong here too.
  2. The BLANK sheet — the one you print before writing anything. Up to 40 pages and 60 MB.
  3. A PDF or a scan. A skewed or faint scan is straightened and checked first; if a page cannot be read properly it is refused and told which one.
  4. Never a filled copy: handwriting or ticked boxes on it and it is refused, because that copy would print underneath every patient from then on.

The system does the tedious part

It reads your paper and puts the boxes on it by itself: each place where something can be written or ticked. You do not draw anything. Press a box and it tells you what it thinks it is; one click changes it, you drag it to move it and pull its corner to resize. If one is missing, add it from the palette or by double-clicking exactly where you want it.

Tip

There are boxes the system fills on its own — the patient’s name, their record number, the date, and, if a representative signs, their name, relationship and reason. Drag them onto their line and there is nothing else to wire up.

Saying who signs where

Your paper carries several signature lines and they all look alike, so before publishing the system asks whose each one is: the person filling it in, the patient, or the office countersigning. It then stamps each line with the signature of its owner, and the office line waits for the office. On a sheet the system already knows, it comes pre-assigned and you only check it. If your form is not signed at all, you say so and the portal will not ask for a signature.

Regulatory sheets are checked before publishing

On a sheet the regulation governs, a button lists the pieces your paper has to carry, why, and which ones are missing. What is already there shows in green; what the standard sheet carries and cannot be found on yours shows in amber. Each missing piece is placed with one click. Until they are all resolved you cannot publish — and if your paper genuinely does not carry one, you say so and that is the end of it.

If the sheet is already ours

A sheet that is already in the system does not come through this door, and it does not need to: it is already yours, it prints on your letterhead and it updates itself when regulation changes. If what you want is to change its wording or add something of your own, open it and use “Edit system form” — that keeps what the law requires, lets you go back whenever you want, and keeps the system recognising the sheet when it comes back scanned. Pushing it through this door instead would turn it into a flat picture of itself and lose all of that, so the system stops you and points you at the editor.

Tip

Official forms are the opposite case: the IRS W-9 and the AHCA sheets are not ours, so they come through this door normally.
